IF YOU ARE A CONTINUING EDUCATION PROVIDER OR WISH TO APPLY TO BE ONE THE FOLLOWING FORMS ARE AVAILABLE.

As a reminder, approved courses are valid only until the end of each licensing period. This means that when licenses are up for renewal, you will also need to apply for course renewal.
For example, if your course was approved in January 2023 – your course would be approved through December 2024. AND, if your course was approved in January 2024 – your course would also only be approved through December 2024.
Additionally, we will NOT accept course applications after the course has been taught. If you do not have course approval, we will not accept your roster. Please make sure you are submitting your requests and waiting for approval prior to teaching your courses.
NOTE: For Online Classes you must have Facial Recognition or Photo record of the person taking the class. They cannot just log in and out to accumulate their hours. Their identity must be verified the entire duration of the class.
